BiotrackOS and TRAIN Partner to Build a 90-Day Heart Health Programme for Structured Cardiovascular Prevention

BiotrackOS and TRAIN are partnering to build a 90-Day Heart Health Programme, pairing BiotrackOS's digital health infrastructure with TRAIN's clinical leadership behind a structured, clinically governed approach to cardiovascular prevention.

BiotrackOS, a connected health intelligence platform built around a single, continuous health record, and TRAIN (TRAIN Health Awareness) have entered into a multi-year strategic partnership to develop the TRAIN 90-Day Heart Health Programme, an integrated remote lifestyle-related disease monitoring platform. The programme is intended to be available to patients referred through participating clinical and institutional partners, and is being designed to support structured cardiovascular prevention and longitudinal personalised monitoring, under TRAIN's clinical governance.

Cardiovascular disease (CVD) remains the leading cause of mortality globally, accounting for nearly one in three deaths. Despite advances in treatment, prevention and long-term risk management remain inconsistently embedded into daily patient care, highlighting the need for structured, digitally enabled cardiovascular support systems.

TRAIN was founded by internationally recognised surgeons:

  • Prof. Dr. Nimrat Grewal, a cardiothoracic surgeon specialising in aortic pathology at Amsterdam University Medical Centers (the Netherlands) and adjunct Professor at the Yale Aortic Institute (USA). Her work integrates translational research, personalised cardiovascular risk assessment, and digital prevention strategies, including her role as co-founder of TRAIN. She is committed to advancing inclusive cardiovascular science, with a focus on diversity in disease presentation and equitable prevention.
  • Dr. Simran Grewal, a double fellowship-trained orthopedic trauma surgeon with a PhD in the effects of surgical trauma on the immune system, has dedicated her career to helping patients build strength, preserve mobility, and prevent lifestyle-related diseases across the lifespan. Through her clinical practice and research, Dr. Grewal advocates for proactive lifestyle strategies that optimise musculoskeletal and metabolic health.

Together, they combine surgical expertise, translational research, and digital health innovation to support structured lifestyle-related disease prevention, especially cardiovascular disease.

Under the partnership:

  • BiotrackOS will provide a secure digital health infrastructure, data-driven analytics frameworks, and wearable data ingestion capability, enabling integration of multi-source health data including biomarker inputs, and where appropriate, research-linked datasets used under TRAIN's research governance framework.
  • TRAIN leads clinical programme design, governance standards, and scientific oversight of the cardiovascular prevention pathways.

The platform is being designed to support structured cardiometabolic data aggregation and cardiovascular risk assessment support, within institution-ready governance frameworks and under TRAIN's clinical oversight. Deployment will occur in accordance with local regulatory, ethical, and data protection requirements.

The TRAIN 90-Day Heart Health Programme is being developed as a TRAIN-branded, clinically governed programme, supported by the BiotrackOS digital infrastructure.

The 90-day pathway is being designed to translate established cardiovascular prevention principles into a structured digital format, incorporating:

  • Baseline cardiovascular risk profiling
  • Structured behavioural activation
  • Data-driven personalisation, developed with clinical input
  • Ongoing tracking of cardiometabolic indicators
  • Structured reporting pathways aligned with clinical oversight

Once complete, the programme is intended to work as follows. A participant begins with an initial cardiovascular risk assessment, receives a personalised 90-day plan combining lifestyle guidance and monitoring, and works with structured check-ins that adjust the programme as their indicators change.

An initial pilot is planned across Amsterdam and India, to be delivered via a connected partner organisation, subject to the regulatory, ethical, and research governance approvals required in each jurisdiction. This first phase will focus on establishing the pilot cohort and validating the monitoring pathway, ahead of the collaboration's longer-term goal of a scalable digital infrastructure to support structured cardiovascular risk management at population level.

Beyond the pilot, further regulatory and ethical approvals covering wider data-driven risk assessment capabilities, ongoing monitoring, and any research-linked data use will be secured as the programme extends to additional institutional partners, in line with the research governance frameworks of the participating hospitals and institutions, under TRAIN's clinical leadership.

James Wildish, CEO of BiotrackOS, commented:

"BiotrackOS was built to provide a secure, scalable digital health infrastructure capable of supporting institution-level deployments. By combining this infrastructure with the clinical leadership behind TRAIN, we are establishing a structured framework for digitally supported cardiovascular disease prevention and long-term monitoring."

Prof. Dr. Nimrat Grewal, co-founder of TRAIN, added:

"Cardiovascular disease is one of the leading causes of death worldwide, but the good news is that it's very much preventable. By making some little lifestyle changes towards a healthier lifestyle, one can significantly improve their heart health. This collaboration enables a digitally supported framework designed to complement existing cardiovascular care pathways and support long-term risk management."
